Late Cancel/No Show Fee
No Show/Late cancellation (less than 48-hour notice) will result in the client being charged the full session fee. **Please note insurance does not cover no-show fees; these are at the expense of the client.**

Payment
You must have a credit, debit, or HSA card on file before your first appointment. If you have an HSA card on file, you must also have an active credit or debit card on file. If your card is declined, all services will be put on hold until your account is paid in full. Your card will automatically be charged your co-pay or deductible the day of service and any remaining amount after your insurance has processed. If you are self-pay, your card will be charged the day of service. If you elect to opt out of billing your insurance, you will be required to sign an insurance opt-out form. If you will be filing OON, please let us know at the start of services, and we will provide you with a superbill.
It is the client's responsibility to know if services are covered by your insurance. Please contact your insurance to find out if you have behavioral health benefits, deductibles, co-pays, etc. You are responsible for any and all service fees that your insurance does not cover.
Good Faith Estimate Notice
You have the right to receive a “Good Faith Estimate” explaining how much your medical and mental health care will cost. Under the law, health care providers need to give patients who don’t have insurance or who are not using insurance an estimate of the expected charges for medical services, including psychotherapy services.
You have the right to receive a Good Faith Estimate for the total expected cost of any non-emergency healthcare services, including psychotherapy services. You can ask your healthcare provider, and any other provider you choose, for a Good Faith Estimate before you schedule a service. If you receive a bill that is at least $400 more than your Good Faith Estimate, you can dispute the bill. Make sure to save a copy or picture of your Good Faith Estimate.
